WHICH TYPE OF EYE EXAM IS FOR ME?

ROUTINE EYE EXAM

A routine eye exam is a basic screening to check vision and update your prescription.  Vision plans only cover a routine exam and may cover materials for eyeglasses and/or contact lenses.

Vision plans DO NOT cover medical eyecare (the diagnosis, management, treatment or follow up visits of eye problems).  An example of a vision plan would be Davis Vision or VSP.

If you have both types of insurance plans it may be necessary for our office to bill accordingly.  If some fees are not covered by your insurance, you will be billed for them.  This includes co-pays, deductibles, or non-covered services as allowed by the insurance contract.  

MEDICAL EYE EXAM

Medical insurance plans may cover certain medical diagnoses, testing, treatment, and follow-up visits for eye conditions such as:

  • Allergies

  • Cataracts

  • Dry eyes-itching, burning, tearing

  • Eye infections

  • Glaucoma

  • Injury to eyes

  • Irritated eyes

  • Macular degeneration

  • Pain in the eye

  • Red eyes 

  • Sudden loss of vision

  • Referral from an outside physician

Medical insurance MUST be billed if the doctor determines you have any eye health problems or systemic conditions which have ocular complications.  

Medical insurance plans include Medicare, Blue Cross & Blue Shield, United Healthcare.

*We are NOT Medicare Advantage providers but can help you submit your out of network claim should you choose to see our doctor.

Please have your insurance card available when calling our office.  One of our friendly opticians can help you understand your specific coverage and discuss further financing options with you.

WHAT TO BRING TO YOUR APPOINTMENT:

-Photo I.D.

-Insurance card(s)

-Completed patient forms (New Patients)

-List of medications (if you have one)

-Most current eyeglasses (driving, reading, or progressives)

-Actual boxes/ Rx of current contact lenses 

*Copays will be collected at the visit

-Head towards Fountain Park

-We are in Plaza Fountainside Mall near Euro Pizza Cafe

-Our office is adjacent to the famous Fountain

Directions from Scottsdale:

-Take 101 freeway 

-Exit Shea Blvd., head east on Shea 

-Turn left on Saguaro Blvd.

-Continue north for about 1.5 miles
